Please select the appropriate form and print it out on your printer. Fill it out as best as you can before our first meeting. All new patients should fill out the Patient Information and Informed Consent forms.
Please complete the Release of Information form if you would like me to contact a previous therapist or physician.
The Informed Consent form is to provide the patient with an understanding of what to expect in the therapy process and how they are protected by the laws and ethics of the profession. If you have any questions or concerns, please discuss with Dr. Kimberley Taylor at your initial meeting. |
